KINDERGARTEN REGISTRATION PACKETS AVAILABLE IN THE ELEMENTARY OFFICE

Packets are now available in the Elementary Office.  Our hours are 8:00 a.m. to 4:30 p.m., Monday through Friday. 

If your child turns 5 on or before September 30, 2012, you are invited to register your child at the Waynesville Elementary lobby, 659 Dayton Road, on the following dates:

Please bring the following documents that are required for enrolling your child (your child does not need to be present):

·         Child’s OFFICIAL BIRTH CERTIFICATE (from the Bureau of Vital Statistics).  We cannot accept the child’s hospital record.

·         Child’s SOCIAL SECURITY CARD

·         CUSTODY PAPERS, if applicable, identifying the custodial/residential parent

·         Parent or Guardian DRIVER’S LICENSE

·         PROPERTY OWNER AFFIDAVIT AND DISTRICT RESIDENCY CONFIRMATION FORMS, both forms must be signed and notarized

·         PROOF OF RESIDENCY (one of the following – see enclosed sheet with residency/enrollment requirements):  Auditor’s Property Summary ŸLease/Rental Agreement ŸPurchase ContractŸ

·         Copy of DeedŸSettlement StatementŸLand Contract

·         ENROLLMENT, TRANSPORTATION and HEALTH FORMS

YOU MUST HAVE PRESENTED ALL OF THE ABOVE PAPERWORK

TO BE ELIGIBLE FOR A SCREENING DATE IN MAY AND ENROLLMENT IN AUGUST

Kindergarten screening will be scheduled on April 19 and April 20, 2012, at the First Baptist Church in Waynesville.  During that time your child will be screened by one of the kindergarten teachers.  You will be notified of your child’s screening results and his/her placement in one of the kindergarten programs.
